and Meter Installation Requirements Guide 01.25 APPLICATION FOR SERVICE NEW SERVICES The first step when applying for service is to contact GPU Energy. There are three ways to contact GPU Energy. Application should be made early to determine if service can be furnished from existing facilities, or if a line extension will be required, and what voltage is available. The three methods to contact GPU Energy are: 1. Application for service can be made by calling the following numbers. GPU Energy will provide you with the Notification Number via telephone call.
2. Application for service can be made by faxing the last two pages of this requirement with the information filled in to the following numbers.

In order to assure prompt processing of the customer's data for establishing service, the applicant should be prepared to furnish all the information indicated on the check lists shown on the following pages, including the LOT AND BLOCK NUMBERS. Once the process is started a confirmation letter is issued. If there is incomplete information the request to provide it will be included in the confirmation letter. It is important for the Company to have complete, detailed information regarding the customer's anticipated load, so that facilities can be properly designed to meet the customer's electrical service requirements. A unique identification number, referred to as the Notification Number, has been incorporated into the service application format. This identification number will be assigned to each job performed by an electrical contractor, and it is to be used as a reference when making inquiries about the status of a job. A notification number is issued for each meter at a location, whether it is for new service or work to an existing service. For example, if service is required for an apartment building for 8 units then 8 notification numbers would be issued. It is also intended that the Notification Number will be used on the electrical permit and cut-in card to further enhance coordination and identification. In residential subdivisions, a unique Notification Number will be assigned to each lot, based on the lot and block numbers. After the application for service has been received and accepted by the Company, it will be forwarded to the appropriate Work Location. A representative of that department or other authorized individual may contact the applicant, if necessary, to discuss the service meter location, and provide other information associated with the installation. If a line extension is necessary, the applicant will be required to provide three copies of appropriate site plans, an executed right-of-way agreement, and payment of any applicable extension charges. The Company will schedule construction work upon receipt of these documents, and completion of site preparation work, as stipulated in the terms and conditions on the line extension agreement. The service will be connected and the meter set only if the installation complies with Company service specifications, the customer's wiring has been approved by an authorized inspection agency, and a "cut-in card" has been received by the Company. Cut-in cards should be hand carried or mailed to the Company by an authorized inspection agent. The Company will accept a facsimile ("FAX") of the cut-in card; for further details, contact the appropriate Customer Operations Center. The Company's service reinspection waiver policy is not applicable for new service.
